Case Managers with Professional Case Management (PCM) build purposeful relationships with our patients and their families as they provide assistance in daily living situations, attend to each patient’s unique preferences and individual needs.

The Senior Case Manager

  • Utilizes critical thinking skills to create patient care plans based on objective assessment of client needs.
  • Serves as an educational beacon for current and incoming staff- training on company policies, processes, and procedures.
  • Possesses strong communication and interpersonal skills
  • Acts as a patient advocate to clients, families, and physicians to promote personal safety and a safe environment for clients and coworkers
  • Provides oversight to all area Case Managers
  • Evaluates all clinical documentation for accuracy, ensuring client homecare needs are met

Compensation: $36.00/hr

QUALIFICATIONS

  • Graduate of a state-approved school of professional nursing.
  • BSN preferred
  • Current, unrestricted license as a Registered Nurse in the state(s) of practice
    • Minimum of two (2) years of nursing experience including one (1) year in home care or closely related field.
    • One (1) year of supervisory and/or case management experience preferred
    • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair
    • Good driving record
    • Current CPR certification &  TB PPD Proof

To perform this job successfully, an individual must have a minimum of two years relevant experience in quality improvement in a medical setting. The ability to generate, analyze, and report data to drive improvement is a must and electronic health record experience is preferred. The individual must be computer literate, able to work collaboratively with diverse and varied individuals, and have excellent writing, research, and analytical skills. In state travel is required approximately 25% of the time and a valid driver's license and an acceptable driving record are necessary. Reno – REMSA Nurse Health Line Manager
This position manages the Nurse Health Line and the nurses and call takers hired to work there. The Nurse Health Line is co-located with the REMSA/RASI 911 Communications Center and operates 24 hours per day, seven days per week. The goal of the program is to improve access to appropriate levels of quality care and treatment and reduce total patient cost for non-emergent callers to the 911 system and to the Nurse Health Line. This position requires strong leadership qualities to manage daily operations of the Nurse Health Line, the ability to make critical decisions based upon objective findings; ability to handle customer and employee complaints using established guidelines and common sense; excellent written and verbal skills; ability to adapt to a variety of working conditions and situations; intimate knowledge of company policy and procedure; and the ability to work as an RN on the NHL.

Qualifications/Experience:
Registered Nursing License in Nevada required. CA RN withing 3 months. At least 5 years' experience as an RN in an ER or acute care setting or 2 years of experience providing nursing care/instructions in a telephonic setting, or any combination thereof. At least 4 years of direct experience managing a team of employees. A track record of excellent interpersonal and communications skills is paramount.
